Foreward Conquering Your Fibromyalgia is an essential contribution that should be devoured by people who have fibromyalgia and by physicians who treat them. At least four million people in the United States suffer from fibromyalgia (FM), a syndrome encompassing varying and fluctuating levels of pain, fatigue, sleep disturbances in addition to major difficulties with memory and concentration. There is scant understanding of the condition and certainly no unified approach to treatment. Most people with FM face resistance from skeptical doctors many of whom are convinced that FM is a fictional disorder endorsed by individuals seeking to obtain disability or have other nefarious motivations. Even sympathetic clinicians struggle to find meaningful and long-lasting treatments that improve their patient’s sense of well-being and daily functioning. In his book Michael Lenz describes the endless obstacles encountered by many FM patients. This labyrinth needs to deconstructed as it leads to patient and physician frustration and subjects too many FM sufferers to unnecessary procedures and dangerous medications. A typical example. An individual with complaints of joint pain, chronic fatigue and brain fog initially presents to their primary care physician. After months of various and unsuccessful attempts with non-steroidal anti-inflammatory medications and a few visits to a chiropractor a referral to a specialist is triggered. The rheumatologist might focus solely on the patient’s arthritic knee pain and begins a series of steroid injections. More often than not these injections provide transient relief to the joint but the treatment leaves the patient’s vexing problem of fatigue completely unaddressed. An orthopedic surgeon may be brought in either by the rheumatologist or directly by the frustrated patient. The surgeon pronounces knee damage and may recommend a total knee replacement (TKR)., Lenz informs us that in 20 percent of cases, the exasperating pain continues, even after such surgery. The orthopedic surgeon cannot understand the poor outcome: the patient still feels deep discomfort but the X-rays show a perfectly replaced joint. Physical therapy is consulted and 12 sessions of hard work does not yield sustained benefit. Increasingly desperate, the physician sends the patient to a pain management expert, who might recommend a muscle relaxer or even opioids—both sedating and potentially addictive drugs which typically cause more problems than they solve. Still no relief? A consultation with a psychiatrist occurs next; she might conclude that the panoply of symptoms is a result of the patient’s dependency on opiates or an underlying anxiety and depression. All too often simply detoxifying the patient or adding an antidepressant does not address the ongoing physical and cognitive complaints. At this point the patient is angry, doctors are contradicting their colleagues and many clinicians wash their hands and slip away. The patient is left with greater sense of hopelessness. Dr Lenz describes this all too familiar scenario, “For many suffering with FM, they are like an innocent person who has been kidnapped, blindfolded, and tortured by a mysterious and cruel assailant for no apparent reason and with no apparent hope for rescue. Even worse, no one else may believe it is happening because they can’t see anything physical wrong with them [the patient].” Using multiple patient vignettes and colorful analogies to illustrate his important points, Dr. Lenz provides examples of the diagnostic and treatment challenges facing the FM community. Dr. Lenz considers all aspects of the syndrome and offers self-tests for fibromyalgia that can be easily completed. These surveys, when routinely administered, alert the clinician to the possible presence of FM. A working pediatrician and internist, Dr. Lenz offers a unique chapter on children who suffer from this disorder, and whose parents may be told the children have “growing pains”—another variation of “it’s not a real problem.” He views the condition across throughout the lifespan, a perspective absent from most existing discussions of FM. From this unique clinical vantage point Lenz identifies that FM is often associated with other misunderstood disorders particularly attention deficit/hyperactivity disorder (ADHD) and chronic fatigue syndrome (CFS). To this reader, Lenz’ chapter on attention-deficit/hyperactivity disorder (ADHD) is or particular import. He notes that these individuals may have an altered response to pain threshold than others and that childhood ADHD may be a risk factor for FM Yet, as I have found in my own studies, one of which is cited in this work, their pain is often responsive to stimulant medications for ADHD. The stimulant increases the person’s ability to concentrate and at the same time, it decreases their response to pain stimuli. The underlying condition question as to whether ADHD and FM/CFS are two halves of the same walnut is yet to be resolved. Beyond reviewing the growing literature regarding the use of non-opiate central acting medications for FM, the author endorses a holistic approach combing with exercise, medication, and thoughtful nutrition. Lenz is at his best when describing the role of a plant-based diet in this complement. These are the sections of the book without peril. Conquering Your Fibromyalgia is a unique addition to the literature and knowledge about fibromyalgia. I am eager for patients and their providers to integrate this well weaved and highly researched book. Internists, family doctors, and pediatricians, as well as to specialists such as rheumatologists, orthopedic surgeons, neurologists, and psychiatrists will be enhanced by the author’s mastery. In years past, depression was often ignored as a “real” problem until finally the medical community acknowledged that this mood disorder is a valid problem affecting million and needs to be treated. Now it is time for physicians to step up and realize fibromyalgia is a similarly debilitating medical problem that strongly merits diagnosis and treatment. Conquering Your Fibromyalgia is an important contribution to bringing both doctors and patients up to speed on this profoundly serious problem today, offering them helpful solutions to their perplexing problem.
