I want to re-design a survey template using Google Docs that we use in communications with our clients. We call this a Candidate Submittal Package.
Currently, the format is in Microsoft Word and we are migrating to Google Docs. So, we want this template designed for Google Docs. I have attached the file we currently use.

The template will integrate with Salesforce have the answers to survey questions auto-populate. So, space for responses to individual questions needs to expand to accommodate paragraph text while protecting the design.
We use square borders around the tables in our original design and I would like to see a cleaner looking design with either rounded corners or more white space.
We are very open-minded on the design as long as it matches with the look and feel of our website and is appropriate for a high end executive business service.
