Kaleidoscope Care is hired by hospitals to create + deliver personalized care plans for patients that explain what they should do after they leave a hospital. Within 24 hours of a patient being discharged from a hospital, we send them a text message that includes a URL to a personal care plan they can view on their phone. Their care plan includes a step-by-step guide of what prescriptions they need, what additional doctor appointments to make, how to connect with their medical team if they have questions or an emergency, as well as brief questionnaires and embedded educational videos. Patients easily and quickly follow their care plans, and as a result do not have to be readmitted to the hospital as frequently.

We want a new, mobile-first design for our care plan template. The design should be similar to an app, not a webpage or webform, even though it will be viewed on a browser. The overarching goal of design should be to drive user engagement all the way through their plan until they complete every step/screen. We want a clean design and to present only the minimum necessary information for each step so that the patient is not overwhelmed and can quickly understand and take the desired action. The included information will be personalized for each patient, but the sections of every care plan will be the same. The user should be able to scroll and easily get pulled through each step to the following step - we will not use a nav bar or menu, and want to remove any work or extra requirements from the user. Functionally, the flow of the user/patient is 0. The user clicks on a shortened link in an SMS and is taken to the care plan via their mobile browser. 1. Patient sees an identify verification screen (with our client hospital's logo included to build trust) and enters their DOB to continue. 2. They get a quick overview of the care plan and how it works 3. They are taken through completing a 6-step process immediately (1. filling out a survey of how they feel; 2. reviewing prescribed medications and calling their pharmacy; 3. calling to schedule their next appointment; 4. reviewing pre-scheduled appointments made by their doctor; 5. complete questionnaire on substance abuse and financial well-being; 6. Watch embedded health education videos ) 4. The land on a Confirmation & Thanks for completing screen that signifies they've finished successfully. The user should be able to track their progress and go back to review previous answers, if necessary. They should be able to click on phone numbers and addresses listed in each screen to call or view the location on maps. We will need a design for desktop, too (90% of users will view on their phones, 10% on a desktop). The attached screenshots from our MVP design are only to make the workflow clearer - they are not design guides or desires. Please be creative!
Our care plans are designed to be beautiful, engaging, easy to understand and simple to follow - we want people to be able to quickly take the most important step in their care, rather than see all of the details. More dynamic, modern design is encouraged. Many of the patients we serve are not tech savvy and are intimidated by healthcare, and have been signed up for our service by someone else (their doctors/hospitals). So, we need great design to help make it clear why they're receiving this care plan on their phone and what steps to take. Big text and universal iconography are helpful. Our goal is to recreate the design of an app, without requiring people to download an app. The overall feel of the design and text should be warm and personal, as if they are being supported by a personal health trainer. We do not want the design to resemble a webform.
