We are a technology company that helps people create and share their personal end of life medical plans digitally.

Please refer to the designer pack provided. We are only looking for the slides 4, 6,7, 8, 9 12 and 13 for this competition. We will work with the winning designer for the rest of the slides. The attached Designer Pack zip file contains all instructions, a complete Powerpoint with all text content, slides logo's, fonts and example charts, icons and images already created. In most cases the charts and icons are examples the type of icon and chart style for you to apply your design skills. Some charts are the final product just requiring style applied. We would like the final as a current Powerpoint PPSX 16:9 format with clean slide masters and editable charts and icons.

We have done most of the hard work and have fully specified the style, design and layout desired. We are seeking your style and design skills to be applied using the design brief provided.
