I need a 2-column webpage template created to generate subscriptions for my "Death of Forced Continuity" report, along with a striking, visual representation of the report itself (i.e. digital cover).
The basic layout should be something like this:
...but obviously more graphical.
The headline at the top left should read: "Free Report: The Death of Forced Continuity" and there should be a graphical representation of this report to the right of the headline with arrows pointing to an email subscription form below the report. (Please use the same basic "call to action" text that's in the example page above.)
The webpage should leave room below the headline for HTML text (to be entered by me). Please paste in some filler text so I can see what it will look like.
It's VITAL that the email subscription form be "above the fold" in the design. In other words, when the design is converted to a webpage, the form should be visible without scrolling.
Above all else, the text on the site MUST be readable, and the subscription area where I'm asking for their email address should stand out.

As for the design itself, I'd like it to be fixed width, centered, with tables. I needto be able to tweak it a little, and I'm not skilled enough to editfull-blown, tableless CSS.
